As part of our commitment to the provision of exceptional hospice palliative care and end-of-life care in our region, we will continue to support the streamlined palliative referral process through the use of the North Simcoe Muskoka (NSM) Palliative Common Referral Form through Ontario Health atHome which connects patients with life-limiting illness to seamless and high-quality care coordination.

All Case Management requests received by the NSMHPCN moving forward will be transitioned to Ontario Health atHome with no disruption for patients or to the healthcare system. This transition will ensure patients and their families continue to receive timely, equitable access to high-quality care palliative care supports through Ontario Health atHome. Please note, the NSMHPCN case management referral were removed from circulation effective August 15, 2023.

The NSMHPCN will expand and continue to provide the Palliative Pain and Symptom Management Consultant Program (PPSMC) which focuses on the provision of a palliative approach to care in primary care through case-based learning, joint visits, mentorship and education to further develop and enhance the skills of front-line service providers.
