The Advance Care Planning Champions of North Simcoe Muskoka (ACPC) are a collective of individuals from various organizations across our region who are passionate about educating the public through ACP conversations and identifying their SDM.

Our Mission: “That everyone in Simcoe/Muskoka over the age of 16 will engage in Advance Care Planning conversations with their substitute decision maker (SDM) and their friends/family members.”
