Keeping Us Connected

During the pandemic, the North Simcoe Muskoka Hospice Palliative Care Network’s (NSMHPCN) community programs have been adapted to be delivered safely through a variety of virtual platforms.  The “Keeping Us Connected” program provides ongoing telephone support for isolated seniors and at-risk populations who are living with a life-limiting illness.

Our regular calls serve as check-ins, needs assessments, security checks and the opportunity for friendly social engagement. We also focus on offering people a chance to be heard by providing emotional support, companionship and a sense of community. We work together to reduce feelings of isolation and loneliness, provide positive coping skills and offer referrals to community services. This includes wellness supports such as:

  • Ensuring the safety, health and well-being of palliative individuals living alone in the community in the absence of routine engagement with friends and family,
  • Virtual social & wellness programming,
  • Telephone reassurance,
  • Security checks and visits, and
  • Supports for caregivers including group meetings and one to one supportive counseling.
