Hospice Palliative Care Ontario (HPCO)

Hospice Palliative Care Ontario is a provincial association of hospices and palliative care providers, professionals, and volunteers throughout Ontario. They envision a future where every person and family in the province of Ontario can quickly and easily access the finest standard of hospice palliative care when required.

NSM Regional Palliative Care Network

The Regional Palliative Care Network of North Simcoe Muskoka provides planning and strategic implementation of palliative care resources and services at the local level to ensure a system-wide approach to palliative care that’s patient-focused.  It is made up of local partners such as home care agencies, hospitals, long-term care, primary care, hospices and other organizations to improve palliative care for patients and families.

Ontario Health

Ontario Health is an agency created by the Government of Ontario to connect, coordinate and modernize the province’s health care system. They work with partners, providers and patients to make the health system more efficient so everyone in Ontario has an opportunity for better health and wellbeing. They oversee health care planning and delivery across the province to build a person-centred health care system.

Ontario Palliative Care Network (OPCN)

A partnership between Cancer Care Ontario (CCO) and the LHINs, funded by the Ministry of Health and Long-Term Care, which is dedicated to help deliver on Ontario’s commitment to palliative care. Their goal is a hospice palliative care system that puts the patient and their family at the centre of every decision, and that is available to all Ontarians who need it regardless of age or disease type.
