The community support services (CSS) agencies that make up the Network share guiding principles to work with other health care and community partners and planners to coordinate and integrate our local health care system to ensure access to the right service at the right time in the right place to support the needs of individuals with ongoing community support needs and their families in North Simcoe Muskoka.

    The Alzheimer’s Society of Muskoka provides meaningful support that helps people live well with dementia. Providing supports for people with Alzheimer’s disease and other dementias and their caregivers with a mission to provide meaningful support that helps people live well with dementia. Programs include, but not limited to:

    Brain Injury Services Muskoka Simcoe provides community services to adults and families living with the on-going effects of an acquired brain injury.  The agency offers individual and group programs throughout the County of Simcoe and the District of Muskoka, and Adult Day Programs in Barrie, Collingwood, Midland and Orillia.  All programs focus on individual independence and the development/maintenance of functional living skills.

    Breaking Down Barriers is an Independent Living Resource Centre providing community-based supports, services, resources and programs for adults with diverse disabilities. Throughout Simcoe, Grey and Bruce Counties our independent living skills training, peer support and interactive initiatives enable individuals with disabilities and their support network to participate without barriers in all aspects of society. As the regional contact for the Direct Funding Self-Managed Attendant Services program, we also help adults with physical disabilities become employers of their own attendants. Our Adult Day Programs are based in Collingwood and as we introduced our programs and services to social media platforms, we are fostering a strong, vibrant and engaged community. These virtual activities continue to break down social barriers by providing greater accessibility.

    Chigamik Community Health Centre is a non-profit, community-governed, charitable organization that provides a combination of primary health care services, community programs and community development initiatives that are culturally-relevant, holistic, and available in English and French – all under one roof. Our priority populations include Francophone and Indigenous communities, LGBTQ and individuals facing barriers to care living in North Simcoe Muskoka.

    Deaf Access Simcoe Muskoka is a community-based non-profit organization providing individualized supports, recreational programming, and accessible resources for deaf and hard of hearing people, those experiencing sudden or progressive loss of hearing or speech, and their families from across the region.

    Hospice Huronia provides programs and services for those in our community with a life-limiting illness, and their families. Our grief & bereavement programs, caregiver support and most recently our 5 Bed Hospice Home are available at no cost for those who need us thanks to a generous community & the Ministry of Health & Long Term Care.

    For the dying and the bereaved, Hospice Simcoe provides compassionate, expert care and services to ease the pain – before, during and after their journey. Hospice palliative care addresses the specific physical, psychological, social, spiritual, and practical issues, and their associated expectations, needs, hopes and fears on an individual basis. The individuals and families we serve are at the centre of care. We are here to assist, educate, support and guide you and your family through your journey. Hospice Simcoe provides 24-hour palliative care at our 10 bed residence along with a variety of bereavement support groups for anyone experiencing the loss of a loved one, as well as day programs for those living with a life limiting illness and their caregivers.

    March of Dimes Canada supports Older Adults to live longer at home in the communities of North Simcoe and South Georgian Bay. We provide support with Homemaking services (light housekeeping, laundry), Assisted Living Services (daily support with personal care, hygiene, medication supports) and Low Acuity Services (personal care and hygiene support for 1-2 hours per week).

    March of Dimes Canada provides Brain Injury services to Adults living in the North Simcoe Muskoka region. We provide case management and individualized support in collaboration with behavioural consultation.  We provide peer groups offering a variety of social, recreational and life skill opportunities.

    Huntsville Meals on Wheels – Provides weekday, lunch-time delivery of hot, nutritious meals at a reasonable cost to anyone needing this service, delivered by friendly volunteers.     Muskoka Seniors is a not-for-profit, registered charity supporting seniors in the North Muskoka region.  Our mission is to help seniors live at home by providing a variety of services supporting independence.

    NSMHPCN is committed to providing dedicated, compassionate, holistic palliative approach to care for all. The team consists of Palliative Pain and Symptom Management Consultants (PPSMC), Social Worker, and Bereavement Services Coordinator, who provide support to those living with a life limiting illness while focusing on respect, collaboration, inclusivity and integrity. NSMHPCN also provides educational opportunities and in-service presentations to other healthcare providers within the region through a wide range of workshops and conferences.

    NSM Specialized Geriatric Services are hospital and community-based health care services supporting frail older adults and their caregivers. A team of health care professionals supports the assessment, diagnosis and initial management of issues commonly seen in older adults with frailty. Team members include geriatricians, geriatric psychiatrists, nurses, social workers, occupational therapy, physiotherapists and pharmacists.

    VON is a highly-trusted non-profit organization that works with our clients, employees, volunteers and partners to provide innovative clinical, personal and social support to people who want the comfort and peace of mind of living in their own homes and communities. At the VON Simcoe County site, our support services include adult day programs, in-home respite, SMART exercise programs & Healthy Aging education, and the Home at Last program.
